We ask that all new patients complete a screening form before the initial visit. The form can be picked up in office, emailed or faxed. Please contact us at 281-534-1444 to have the form sent or with any questions you might have. We accept most public and private insurance plans. We also offer special pricing for cash-paying patients for most procedures.
